Introducing Our NEW Population Health Website – An Answer to a Familiar Question
What is Population Health?
Population health is many things. It is a framework to improve value in health care delivery. It is a payment model. It is an academic field of inquiry and scholarship. It is a team of teams—from Brigham Health, Mass General, our community and specialty hospitals, and our vast physician network— committed to improving quality and bending the cost curve.
Over the past decade, we have collectively built a robust infrastructure with countless lessons learned. How do we tell this story? Given its complexity, it is hard (and unfair) to answer this question with a simple answer. It is important for us to have ways to effectively communicate the breadth of the work – both internally across our network and to the broader health care community across the globe.

Partners Population Health consists of many teams across Partners HealthCare, dedicated to researching and redesigning clinical care in a way that focuses on the whole patient. We design population health management strategies, programs, and tools to help improve outcomes, patient and provider satisfaction, and reduce overall health care costs.
Population Health care redesign programs are shaped and delivered by the Partners care teams on the ground–from physicians and nurses, to social workers and community health workers, as well as unique support staff like recovery coaches and behavioral health support specialists.
By leveraging the breadth and strength of the Partners HealthCare family, our population health management initiatives are able to impact the lives of over 700,000 patients across Massachusetts and beyond. Our central team works with local teams across the Partners system to develop, implement and manage a system-wide value-based care strategy for all patient populations.
