Population Health care redesign programs are shaped and delivered by the Mass General Brigham care teams on the ground–from physicians and nurses, to social workers and community health workers, as well as unique support staff like recovery coaches and behavioral health support specialists. By leveraging the breadth and strength of the Mass General Brigham family, our population health management initiatives are able to impact the lives of over 700,000 patients across Massachusetts and beyond.
In addition to its two academic medical centers, the Mass General Brigham system includes community and specialty hospitals, a managed care organization, a physician network, community health centers, home health and long-term care services, and other health care entities.
